Observations:

Based on review of agency information packet given to consumers, and email from EMP1, the agency failed to provide the consumer with required information prior to start of service for one (1) of one (1) consumer (CR1).

Findings included:

Review of email from EMP1 on December 22, 2022, at 1:30 p.m. showed agency developed a consumer information packet given to consumers prior to commencement of services that contained incorrect information concerning who to contact at the Department (717-783-1379) for information about licensure requirements for a home care agency or home care registry and for compliance information about a particular home care agency or home care registry. The agency listed an incorrect number 717-783-8665.

The agency was to have developed a consumer information packet containing correct information by 9/19/2022 according to its plan of correction from survey completed 8/19/2022.

Per email from EMP1 reviewed on December 22, 2022, at 2:23 p.m. the agency added one new consumer (CR1) on 9/29/2022 who would have received the packet containing the incorrect number for who to contact at the Department.
